Standard Sudoku rules apply. Little killer, the number along the indicated diagonal must sum to the given total. Numbers must not repeat in a killer cage. The numbers in the cage must sum to the given total in the top left.

I was just starting to debate whether I had gotten in over my head when I saw pieces fall into place. A nice one that ended up NOT being the headache I feared.
