Apply classic sudoku rules.
The small numbers in the upper left corner of the cages indicate the sum of the digits enclosed in that area. Within each cage all digits must be different and they must be placed in a way that they are all orthogonally connected in increasing order, such that they form a worm-like path of one cell width (i.e., like a thermometer but with no diagonal steps).

The idea of 'thermal killers' originates with Madmahogany. There is also an exemplar in Richard's SVS series.
